It might take 24 hours for the App to be pushed out to customers NowTV branded devices (so be patient).
You could try a system update in the menu settings just to see if the App is idle there waiting to go into the Apps Store section on your box / stick.
Just one word of caution the old white nowtv boxes will not be receiving the Disney + App from what i have heard and currently it's only being rolled out to UK customers where Republic of Ireland customers will have to wait a little longer according to NowTV over on their Twitter feed.

You need a device supported for the Disney+ app to watch their content.

@gavs82008 gave you the short answer, and he’s quite right.
But the Stick/App confusion comes up quite a bit, so here’s the long answer.
if it’s a device, like the Now Stick was, then it will have various apps on it, including the Now app, and quite possibly Disney+.
But if the device (which might be a TV) has apps on it, and one of the apps is the Now app, then there are no apps within the Now app.
Because the TV, with various apps on it including Now, is analogous to a Roku device or a FireStick with the Now app on it.
In the dying days of the Now Stick, Now did what I thought was a fairly bewildering thing, which was to make it look as though the apps on the Stick were nested under the Now app, engendering a confusion that still persists; but this wasn’t ever really the case, as you could see if you arrived left from the Now screen, whereupon the true app structure was revealed; nothing nested.
