Cancelled my voucher 2 months ago and am still being charged - how do I get a refund?

From the top of the page, click on "my account" and select "my passes". You should see a link for an incorrectly charged form.Screenshot_2018-05-09-22-20-40.png

I’ve noticed that now tv have been taken 9.99 twice put my bank and I am unable to get in contact with them has anyone got any suggestion
