Wall mounted phone connector issue

Hey all,

I have an issue with the phone connector - the one that's on the wall that you plug your cable that runs to the router and you also plug your phone into. It has come off the wall (seems to have only been stuck up with some double sided tape) and it has pulled some of the external cable inside. Further to that, when it came off the wall it pulled some of the connecting wires into the box loose. Whilst I'm still connected at the moment I fear it could easily become detached.

This was a brand new line put in when I first joined Now TV less than a year ago. It needs someone to come and reconnect it properly and secured to the wall. How do I report this?
