With any of the NowTV boxes you pay a one off purchase price for the box and then you can either buy voucher passes or pay a monthly subscription fee for whichever NowTV passes you want to watch (i.e. entertainment, cinema, kids etc)
If you currently have a NowTV account with your white box, all you need to do is sign into the new box with your existing username details.
If you don't need the inbuilt Freeview TV tuner on the NowTV smart box, you can save a few quid by picking up the NowTV std black box with the added bonus of a voucher pass in the box which are currently priced around the £15 to £20 mark from places like Argos etc.

The cost of running all Now TV boxes are the same, remember you are using the same account and using the same passes, and so you have nothing to worry about on that front.

NOW TV currently offer two NOW TV branded devices to access their content. A NOW TV 'black' box and a NOW TV 'smart' box. Depending on where you purchase these devices from they might come bundled together with one of NOW TV's services, usually it's either the 'Entertainment' service or the 'Cinema' service.
At the time of writing, NOW TV's 'Entertainment' service costs £7.99 per month. And their 'Cinema' service costs £9.99 per month. Which you can cancel at any time.
