I have been trying to cancel my subscription online but when I go into My Passes it directs me to complete my banking details. As I am trying to cancel this why would I have to input these details. I want this cancelled immediately.

It sounds like you're logging in with a different account to the one with which you purchased the pass. I'd suggest checking your account details on a device which you use to access NOW TV.

I don't have the box we used as we moved house. Would the username be my email account. Thanks

If you go to the sign in page you can use the forgotten username and use the email you signed up for the pass with.
