Weirdly, I can’t find any Now instructions on pairing the remote with the TV, but here are the Roku ones; it’s essentially the same remote as the Now enhanced one.

Though the idea seems to be that the box detects the TV over HDMI and then tells the remote what brand to set up for, so you need instructions for this only if the automated process doesn’t work.
What is the exact make and model of your Fire TV?

The TV is a 4-series fire tv by amazon. When I got to remote settings in now tv (smart stick) the window comes up to search for tv but I cannot find it, so I am guessing you can't use it

Don't these Amazon Fire 4 smart televisions have the NOW native App on them ?
Or do you want to use the NOW Stick with this television for some particular reason?
When you go through the remote command wizard in the NOW Stick Settings did you try each code before you are offered a list of manufacturers to pick from ?
I think I read somewhere that these new Amazon Fire Smart televisions are manufactured by TCL (so perhaps try going through the different sound code number prompts one at a time in the NOW Stick remote settings by selecting TCL from the drop down list).
This is the next step up Amazon Omni Fire TV, where TCL from the list worked for the Roku streamers.

when prompted to enter the manufacturer and device to set up, enter Microsoft and Xbox 360 (the IR codes sent from the remote to control the Fire TV use the same IR codes as for the Xbox 360).
Worth a try anyway.
But I’m still not quite sure what you are aiming for here. The Now remote is only going to work the Now Stick, apart from the TV On/Off and Volume that we are trying to help you set up.
The Omni Fire TVs, one step up from the Fire 4s, come with Alexa, so your wife would be able to operate everything on the TV that Alexa allows, by voice alone.
What are you both aiming for your wife to be able to achieve? And have you looked at the One For All range of remotes, and what they could achieve here, either by dint of lightness, if that is the issue, or by having big buttons, if that is?
[NB: I’m trying not to be intrusive here, and I don’t need to know the answers to the above, nor am I expecting you to share here, necessarily. Just setting out a couple of questions for you to ask yourselves, to help you be sure that you have covered all the bases in working towards the best solution you can find]

Out of curiosity did selecting TCL (not TCL Roku) from the manufacturers list help with the NOW Stick Smart Remote TV standby and volume buttons when using the Amazon Fire Smart TV ?
Or no joy ?
