Disney+ is working on my NOW TV stick but the sound is very tinny. Other apps on the stick work and Disney+ is working fine on other devices. Have tried a system update and restart with no joy. Any ideas?

What about removing the Disney + App and then reinstall it from the Stick Apps Store ?
Apart from that, maybe have a play around with the Audio Settings in the Stick settings menu (start with stereo and work your way through the options on the list).
The other thing you can try is when playing some content press the Star button on the Stick remote and there should be an on screen option to adjust the Volume Mode (make sure it is off for best results).
I don't know if the Star Button works with Disney + because i don't subscribe to their service, but the Star button option is available when watching NowTV & Netflix content.

@schnapps Thank you. It was stick settings that was the issue. It automates to surround sound which I don't have. Flicking to stereo has sorted it
