Modem giving out

My NowTV modem requires a daily power cycle to work. It breaks down with slow and degraded service if I don't reset it. I've had it for 18 months and this wasn't an issue for the first 16 or so. Any advice on how to get NowTV to send me a new one? I can't find a contact form anywhere.

There plenty of threads that contain the number to call. However @redchiz1 has kindly given you it. 

FYI that I do not work for NOW, just a NOW customer trying to help
