There's been a few people mentioning the same thing recently so I'm not sure what's going on.
Make sure there's one of your four device slots empty on your My Account > Manage Devices page, and if there is then have a word with Live Chat, explain what's happened and see if they can sort something out for you as a courtesy so you can get your box added.
