Without sounding arrogant, but I'd like to start the post by saying that I'm a software developer (iOS) and have first hand direct experience in developing apps for Apple Device. Sorry... it's just to say that I've already verified the issue and I can provide technical detail of it.
So... just to let you aware... your App for the new Apple TV must have some sort of problem in its distribution package. Probably because of a problem in the initial project file.
Basically, when the app is installed on a new Apple TV 4gen set up to also download the same app on other devices with the same appleID, an empty app called KORTV gets installed on the other devices. THIS IS NOT THE KORTV APP AVAILABLE ON THE APP STORE. But rather it looks like an empty App that was mistakenly included in the Apple TV project file.
My guess is that the third party development company to which you commissioned the development of your app, is the same company that also has developed KORTV (or maybe KORTV is part of bskyb... don't know... I didn't look into it).
BTW... Here are a couple of screenshoots that show the app that got automatically installed on my iPhone and how the Bundle Identifier captured by the console log in Xcode when launched, shows that the app belongs to the bskyb domain.
Now the issue is just to figure out if this issue is due to a harmless lack of care of your developers or to a more serious security issue. Not a long ago a news broke online that many developers in China were unknowingly using a compromised version of Xcode to develop apps and I wonder if the issue explained above, is a sign that also your software developers are running the same compromised version of xcode.
