Billed for Sky sports month pass after cancelling it last month.

Hi,

 

Last month I cancelled my Sky sports month pass, and today I've just been charged the full amount again. I've tried finding someway of contacting the team to discuss this issue but cannot find any means of doing so. I have filled an incorrect charge form in, but I'm not sure if it went through properly as it gave me a reference of " Your reference number is :%{caseNumber}". 

Any help would be greatly appreciated..
