Account charges

I’ve just had an email to say I was being charged for a sky sports day pass & a now tv boost, but I haven’t authorised this.

i cancelled all my passes last year & have never had a sports pass.

ive tried to fill out the form, but it keeps disappearing once I’ve filled half of it out. 
please can a refund be looked into?
